Health Coaching

For individuals to achieve positive and sustained health behavior change; studies have shown education (knowledge), support and encouragement are critical componets for success. Health Coaching is a strategy that the AHM RN CCM Case Managers utilize as a tool to promote and support our patient's positive adherence to their care and treatment plans. The goal is to provide the patient with an understanding of and desire to remain at their individual optimum level of wellness.

Health coaching services are provided to our clients as a component of the Case Management, Disease Management, and Maternity Management programs. In addition, our "coaching" team provides clients with stand-alone practical solutions to achieve successful goals with targeted populations.

Examples of some of the coaching strategies utilized:

  • Identify all aspects of the patient's care and treatment plan
  • Assist the patient to identify knowledge deficits related to their illness, disease or medical condition and provide supporting education and informations as needed
  • Identify the individual's barriers to successful behavior change
  • Work with the patient to establish new behaviors to overcome barriers
  • Support and encourage all positive results

Our Health Coaching strategies have produced successful outcomes for individual patients as well as clients that have contracted our services for special projects.

Health Advocate

AHM provides the members with licensed RNs who have the knowledge, resouces and evidence based recommendations to help members make critical medical decisions. Members can call to find answers to health related questions, understand their options, learn the right questions to ask, and identify resouces for themselves and their family's healthcare needs.

Special Projects and Services

AHM contracts on an individual basis for special projects.
