Maternity Management Program

The Great Expectations™ Program provides comprehensive risk assessment and medical management for low, medium and high risk pregnancies. The goal of the program is to reduce the risk of pre-natal complications, pre-term labor and promote healthy outcomes for both mothers and babies.

• The team includes RN Case Managers and Program Management support, specialized in high-risk obstetrics
• Risk Assessments are completed for every participant during each trimester to identify any changes in risk factors and implement interventions to reduce potential complications
• The Case Managers provide pre-natal education and monitoring throughout the pregnancy
• High-risk pregnancies are followed by a Case Manager in a focused Case Management program
• Reports and updates are provided to the Physicians and clients on a regular basis

This Program significantly reduces the cost to the health plan and family for both obstetrical care and newborn care by reducing the incidence of preventable high-cost complications.

The AHM Disease Management (DM) Program targets chronic and episodic conditions that historically have increased healthcare costs and contributed to individuals' lost productivity. Through a comprehansive, multidisciplnary approach, we focus on improving the optimum wellness of individuals identified to be at risk. Our specialized RN CCM Case Managers utilize personalized interventions and proven evidence-based medical guidelines to develop each care management plan. Interventions are based on individual risk status and provide a combination of low and high-touch interventions. AHM deploys best-in-class solutions for its' customers to coordinate and address care while promoting positive change and patient empowerment through patient and caregiver education.
